German Foundation for the Chronically Ill

 

Founded in 2004, the German Foundation for the Chronically Ill is a non-profit, independent foundation with the aim of sustainably improving the care of people with chronic illnesses. This goal is achieved primarily through the development, promotion and scientific evaluation of innovative (digital) forms of care as well as by supporting patient education and care concepts. One focus is on empirical social research on questions of acceptance, motivation and the needs and requirements of various stakeholders. The participation of those affected is of particular importance.

The foundation's work is based on the conviction that cooperation between all those involved in care - from doctors, nurses and therapists to payers and health policy representatives - is an essential basis for improving quality of life of people with chronic illnesses.

The patients themselves also have an important role to play: an open and self-determined way of dealing with the disease is an important success factor for coping with it and thus for the success of the therapy.

It therefore seems only natural that the German Foundation for the Chronically Ill is participating in the CAEHR research project, more specifically in the Use Case “Outpatient care”.

Close cooperation between inpatient and outpatient care is particularly important for patients with coronary heart disease or heart failure. A harmonized treatment strategy can enable early treatment and avoid emergencies. With its expertise, the foundation helped to develop the objectives of the CAEHR research project from the perspective of those affected. It also uses a specifically developed questionnaire to evaluate patient satisfaction and the perceived quality of care and processes. The aim is to determine whether the project has actually improved cooperation between inpatient and outpatient care from the patients' perspective due to the availability of digital health data.

The CAEHR team is delighted to be working with the German Foundation for the Chronically Ill to be able to help this group of patients cope with their illness and improve their quality of life!
